Vista Autism Services utilizes an alternate method of furnishing Form 1095-C, as permitted by Internal Revenue Service regulations. Under this method, Form 1095-C is not automatically mailed to all employees. Instead, current and former full-time employees may request a copy at no cost.
To request your Form 1095-C, please complete the secure request form below. Vista Autism Services will provide your form within 30 days of receiving your request, in accordance with federal requirements.
Note: Form 1095-C provides information about health insurance coverage offered by an employer and may be used for personal tax records.
